Indications/Uses
Non insulin-dependent diabetes (type 2) in adults when dietary measures, physical exercise & wt loss alone are not sufficient to control blood glucose.
Dosage/Direction for Use
Initially 30 mg daily (½ tab of Diamicron MR 60 mg). May be increased to 60, 90 or 120 mg daily in successive steps if blood glucose is not adequately controlled. The interval between each dose increment should be at least 1 mth. Max: 120 mg daily.
Administration
Should be taken with food: Swallow tab & divided halves whole, do not chew/crush.
Contraindications
Hypersensitivity to gliclazide or to other sulfonylurea, sulfonamides. Type 1 diabetes; diabetic pre-coma & coma, diabetic ketoacidosis. Concomitant use w/ miconazole. Severe renal or hepatic insufficiency. Lactation.
Special Precautions
Hypoglycaemia which may occur due to low-calorie diets, prolonged or strenuous exercise, alcohol intake, use w/ other hypoglycaemic agents; or presence of risk factors eg, malnutrition, irregular mealtimes, thyroid disorders, hypopituitarism & adrenal insufficiency. St John's wort, fever, trauma, infection or surgery may affect blood glucose control. Patients w/ G6PD deficiency. Monitor blood glucose & measure glycated Hb levels. Concomitant use w/ fluoroquinolones. Not to be administered to patients with rare hereditary problems of galactose intolerance, Lapp lactase deficiency or glucose-galactose malabsorption. Caution in driving or operating machinery. Renal insufficiency. Severe renal & hepatic insufficiency. Avoid use during pregnancy. Childn & adolescents. Elderly.
Adverse Reactions
Hypoglycaemia; GI disturbances including abdominal pain, nausea, vomiting, dyspepsia, diarrhoea & constipation. Transient visual disturbances. Erythrocytopenia, agranulocytosis, haemolytic anaemia, pancytopenia, allergic vasculitis, hyponatraemia, elevated liver enzyme levels, liver function impairment, hepatitis.
Drug Interactions
Potentiated blood glucose-lowering effect w/ other antidiabetic agents (eg, insulins, acarbose, metformin, thiazolidinediones, dipeptidyl peptidase-4 inhibitors, GLP-1 receptor agonists), β-blockers, fluconazole, ACE inhibitors (eg, captopril, enalapril), H2-receptor antagonists, MAOIs, sulfonamides, clarithromycin & NSAIDs. Increased blood glucose levels w/ chlorpromazine (high doses); glucocorticoids & tetracosactrin; ritodrine, salbutamol, terbutaline (IV). Decreased exposure w/ St. John's wort (Hypericum perforatum) prep. Risk of dysglycaemia w/ fluoroquinolones. May potentiate anticoagulation of anticoagulant therapy eg, warfarin. Contraindicated: Increased hypoglycaemic effect w/ miconazole. Not recommended: Increased hypoglycaemic effect w/ phenylbutazone; alcohol. Diabetogenic effect of danazol.
